Position Title: Residential Case Managers
Catholic Charities of Chemung County is seeking 2 highly organized, responsible and team-oriented applicants with knowledge of local community services to fill vacant Residential Case Management positions within the agency.
The core function of the Residential Case Manager’s is the goal-oriented coordination of care and services for disabled and homeless residents that will facilitate the integration of the resident into the community, using community-based services whenever possible.
The RCM will provide direct services to residents; maintain accurate case file records, progress notes, documentation and reports; prepare assessments, develop and implement individual service plans; assist residents in achieving goals, teach and/or assist residents in developing IDL skills, communication and self- advocacy skills; facilitate resident involvement in community activities; make referrals to community-based services; assist in accessing and maintaining entitlements/benefits; advocate for needed services; assist residents in meeting the obligations of tenancy.
Required: Associate Degree in Human Services and two-years of experience in Human Services or related field, a valid driver’s license, reliable transportation and demonstrated ability to work effectively with at-risk populations and people with disabilities.
