Position Title: Resident Counselor/Case Manager
Primary Function:
Based on a psychiatric rehabilitative approach, the primary function of the Case Manager/Resident Counselor is to provide case management services to those participants of Gateways Community Living Program. Case Management/Residential Counseling duties would include, support, advocacy, linkage, education, and coordination of services for assigned caseload within the unified case management system. The case manager accesses benefits and entitlements, conducts assessments, develops and implements individual service plans, attends meetings, generates, reviews, and maintains records and reports based on the requirements of each individual’s funding source. Working to identify and pursue person centered goals, gradually enabling participants to progress toward a more independent residential setting, offering support and / or rehabilitative services through individual contacts.
