POSITION TITLE: Intake Case Manager

PRIMARY FUNCTION:

To act as the primary support person for all persons entering for support and housing services through the Emergency Services Unit.  CM will assess all clients requesting assistance through program services.  Duties will include a variety of responsibilities directly related to client service. This includes promoting a professional and welcoming atmosphere in the Emergency Services Unit.  CM will assist vulnerable individuals and/or families with short-term emergency needs including but not limited to the following types of situations: utility shutoffs, prescription assistance, transportation, housing, and other. CM Controls flow of clients to in-house service providers so that needs are met in a timely manner.
